ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Provider (PCP) Nurse Practitioner to join their rapidly expanding team. Responsibilities
- Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
- Take full accountability for patient care and outcomes, seeking consultation from specialists when needed
- Engage with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Lead the care team for patients able to come to the office
- Engage with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)
- Have an active role in the management of their center and help cover for other providers
- Supervise, collaborate with, participate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the actions of the APP

- $111,140 - $158,771 Salary
- This position may also be eligible for bonuses or commissions
Requirements
- Current Nurse Practitioner Certification in the State of practice required
- Certification in Basic Cardiac Life Support required
- Board certification by AANP or ANCC is preferred but may be required for certain States
- Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
- 1 year of clinical experience preferred, but willing to consider strong new graduates
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
