Signify Health, part of CVS Health, is seeking a full-time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. Clinicians will provide flexible, comprehensive, and personalized health evaluations directly to people's front doors.

Practice Info

• Primary workplace is the private residence of members
• Visits last about 45 minutes
• Use an iPad and an intuitive clinical workflow for each evaluation
• Not required to prescribe medicine, order lab tests, or alter current treatment regimens

Responsibilities

• Engage with people and help them on their journey to better health
• Provide in-home health evaluations
• Conduct a medication and medical history review
• Perform a physical evaluation
• Conduct additional diagnostic tests if ordered by the person's health plan (e.g., diabetic eye exams, spirometry)
• Travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Compensation

• The typical pay range for this role is: $87,035 - $187,460

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (required for MN, as is the filing of the appropriate attestation with the state of Minnesota)
• Board certification
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
• Multi-State Licenses (preferred)
