Job Summary:
Concert Health is seeking a proactive and empathetic Patient Engagement Specialist to join our team. Reporting to the Associate Clinical Director, this role is a key part of our Clinical Operations team, responsible for enrolling patients into Concert Health’s Collaborative Care program. As a Patient Engagement Specialist, you will use your skills in rapport building, active listening, and critical thinking to guide patients through the enrollment process, helping them access high-quality care while contributing to the company's goals.
This position requires a high volume of inbound and outbound calls, with a strong emphasis on customer service and patient engagement.
Responsibilities:
Conduct daily outreach to prospective patients to enroll them in Concert Health’s Collaborative Care program.
Promptly respond to inbound calls and voicemails, delivering exceptional customer service.
Engage with patients from population health reports and conduct screenings (PHQ/GAD) to establish initial care goals.
Schedule patients for upcoming collaborative care encounters and assist them in understanding their medical benefits during the enrollment process.
Address patient inquiries related to enrollment, billing, scheduling, and other concerns.
Document patient interactions and notes using various electronic medical record (EMR) software.
Participate in post-enrollment follow-up activities to ensure patient satisfaction and continuity of care.
Handle 60+ inbound and outbound calls per day.
Available Shift(s): 9am to 5pm or 10am to 6pm Eastern
Qualifications:
- 2+ years of experience in Collaborative Care / Behavioral Health.
- 1+ year of call center experience preferred.
- Bilingual (English/Spanish) preferred.
- Proficiency with electronic medical records (EMR) software; experience with Salesforce is a plus.
- Strong problem-solving abilities and the capacity to multitask in a fast-paced environment.
- Excellent verbal and written communication skills.
- Patient-focused approach with a proactive and empathetic demeanor.
- Experience in Collaborative Care or direct patient care is a plus.
- Familiarity with healthcare billing or revenue cycle operations is a plus.
